Requests for Certified Verification of Licensure or Official Licensing File Transfer

CERTIFIED VERIFICATION OF LICENSURE

A licensee or registrant of the Board of Psychology (Board) might need a Certified Verification of Licensure/Registration when seeking to obtain licensure/registration in another state or jurisdiction or for verification purposes for insurance/employment.

What is included in a Certified Verification of Licensure/Registration:

  • Licensee/Registrant Name
  • License/Registration Type
  • License/Registration Number
  • Initial License/Registration Date
  • Current License/Registration Expiration Date
  • Current License/Registration Status
  • Effective Date of Current License/Registration Status
  • Current Licensee/Registrant Address of Record
  • Certification of any current or prior disciplinary action taken against the licensee/registrant by the Board.

NOTE: The Board of Psychology does not fill out other state licensing application forms.

The above Certified Verification of Licensure/Registration does not include the following information:

  • Periods of Non-Practicing Status
  • Previous Licensee/Registrant Name
  • Licensee/Registrant School or Degree (Major) Information
  • Information Regarding Licensure Reciprocity, Endorsement, or Certificate of Professional Qualification (CPQ) Information

The Board DOES NOT release the following licensee/registrant information:

  • Licensee/Registrant Social Security Number or Birth date
  • Licensee/Registrant Transcripts, Grades, Supervisory Performance Information
  • Examination for Professional Practice in Psychology (EPPP) Scores or Examination Dates (please request a score transfer from ASPPB to obtain this information, available here)
  • California Psychology Law and Ethics Examination (CPLEE) Scores or Examination Dates
  • Information about reasonable accommodations a licensee/registrant received or possible impairments unrelated to formal discipline that a licensee/registrant disclosed to the Board as part of the licensure process.
  • Information regarding pending (non-public) disciplinary action against a licensee/registrant.

HOW TO REQUEST A CERTIFIED VERIFICATION OF LICENSURE

To order a Certified Verification of Licensure, you may submit the following Verification/File Transfer Request Form. Should you choose not to use this form, you MUST send a letter requesting a Certified Verification of Licensure to the Board identifying all of the following information:

  • Your name and license number
  • The mailing address where the Certified Verification of Licensure should be mailed, and if also requesting an emailed copy, identify the appropriate emails.
  • A $5.00 check or money order for each Certified Verification of Licensure requested

NOTE: if requesting multiple Certified Verifications of Licensure for multiple states, please provide a separate letter for each request. These letters can be submitted with a single check or money order for the total amount based on the number of Certified Verification of Licensures ordered.

Once the fee is cashiered, your request will be queued for processing and will be completed in the order received.  Verifications take approximately 4-8 weeks to be finalized and mailed.

Certified Verification of Licensures DO NOT include copies of your licensing file (if you need this, please request an Official Licensing File Transfer below).

OFFICIAL LICENSING FILE TRANSFER

A licensee or registrant of the Board might need an Official Licensing File Transfer when seeking to obtain licensure/registration in another state or jurisdiction.

What can be included in an Official Licensing File Transfer:

  • Any application filed with the Board
  • Documentation of Supervised Professional Experience used to qualify for licensure

The above Official Licensing File Transfer does not include the following information:

  • Licensee/Registrant Transcripts
  • EPPP or CPLEE score reports
  • Correspondence to and from the Board

 
When sending the Official Licensing File Transfer to any person or entity that is not the licensee/registrant, the Board will redact the following information from the documentation. To order an unredacted Official Licensing File Transfer, you may submit the following Verification/File Transfer Request Form:

  • Licensee/Registrant Social Security Number, Home Address, Phone Number, Email, Birth date, Gender, Photograph, and identifying information (e.g. height, weight, eye color, etc.)
  • Licensee/Registrant Transcripts, grades, supervisory performance information, or other performance evaluations or fitness for duty ratings
  • Any Original Signatures
  • Information about reasonable accommodations a licensee/registrant received or possible impairments unrelated to formal discipline that a licensee/registrant disclosed to the Board as part of the licensure process.

HOW TO REQUEST AN OFFICIAL LICENSING FILE TRANSFER

To order an Official Licensing File Transfer, you may submit the following Verification/File Transfer Request Form. Should you choose not to use this form, you MUST send a letter requesting an Official Licensing File Transfer to the Board identifying all of the following information:

  • Your name and license number
  • The mailing address where the Official Licensing File Transfer should be mailed, and if also requesting an emailed copy, identify the appropriate emails.
  • Indicate whether you would like one or all of the following documentation included:
    • Application(s) filed with the Board (please specify Psychologist, Psychological Associate, or Registered Psychologist)
    • Documentation of Supervised Professional Experience used to qualify for licensure
    • A $10.00 check or money order for each File Transfer requested

NOTE: if requesting multiple Official Licensing File Transfers for multiple states, please provide a separate letter for each request.

Once your request has been input into the BreEZe system, it will be queued for processing and completed in the order received.  Official Licensing File Transfers take approximately 6-8 weeks to be finalized and mailed.
